The Solo Aging Support Plan is for individuals who are managing life well but want a reliable professional to check in regularly, help coordinate care, and step in during emergencies or health changes.

This is a membership-based service with limited availability.

• Monthly in-person wellness check-ins

• Help coordinating appointments and medications

• Support and advocacy during emergencies or hospitalizations

• A trusted advocate who knows your wishes

• Optional help with bills, insurance, and more

• Supporting communication with doctors and specialists

What’s the First Step?
We begin with a Welcome Visit to gather your medical info, review legal documents (like POA), and prepare for hurricane or emergency planning. This visit also establishes us as your backup contact.

Who We Are:

We’re a multidisciplinary team with deep expertise in supporting aging adults through every stage of life. Led by Christine Yancey, a board-certified geriatric specialist in physical therapy and board-certified case manager with over 20 years of experience in home health, our team understands the medical, emotional, and logistical challenges of growing older—especially without available family.​ We work alongside physicians, therapists, attorneys, and care providers to ensure each client’s needs are addressed with professionalism, compassion, and clarity.
