  • Cognitive assessments to evaluate mental and cognitive functioning.

  • Comprehensive home safety assessment with personalized recommendations to ensure the client’s environment meets their unique needs.

  • Discussion of the client’s wishes for aging in place versus moving, with recommendations focused on the pros, cons, and financial considerations.

  • Referrals to trusted community resources.

  • Family meeting to create a collaborative action plan.

  • Education on the benefits of ongoing care management, including continuous support through regular home visits, phone calls, video conferences, and email, along with crisis intervention as needed.
