When an unexpected health event or urgent situation arises, families often need help quickly determining what to do next. First Coast Aging Life Care provides responsive, professional guidance to assess needs, coordinate resources, communicate with providers, and develop a plan for moving forward.
First Coast Aging Life Care is not an emergency response or 24/7 on-call service. For medical emergencies, call 911. We respond to urgent care management needs as promptly as possible, with same- or next-day support.
Crisis Support May Include:

Rapid care coordination: Help arrange home care and other support services when needs suddenly change.

Hospital and ER coordination: Assist with hospitalizations, communication with the care team, and discharge planning.

Rehabilitation placement: Help families evaluate rehabilitation options and identify an appropriate facility.

Transitions home: Coordinate services, equipment, medications, appointments, and other resources for a safer return home.

End-of-life planning: Help families navigate goals of care and advance care planning.
